But even with these formal mechanisms in place, the most&nbsp;<strong>accessible and powerful safety tool<\/strong>&nbsp;remains:&nbsp;<strong>the challenge<\/strong>&nbsp;&#8211; Our next best tool. Test assumptions, check for understanding, and ask for clarification. Never assume that everything will be all right.<\/p>\n\n\n\n<hr class=\"wp-block-separator has-alpha-channel-opacity\"\/>\n\n\n\n<h3 class=\"wp-block-heading\">The Power of the Challenge &#8211; Airline Pilots<\/h3>\n\n\n\n<p>A challenge isn\u2019t a confrontation. It\u2019s a&nbsp;<strong>safeguard<\/strong>\u2014a way to stop the chain reaction of assumptions, miscommunications, and unspoken concerns that can lead to accidents.<\/p>\n\n\n\n<p>Take the aviation industry as an example.
